REPEATED RHINOPLASTY (TERTIARY)

Correction of the shape or size of the nose is a fairly popular plastic surgery, which has found wide application in aesthetic surgery. Currently, experts identify for the rhinoplasty method – open and closed. Open nose surgery is most often used when correcting the shape of the nose with the use of grafts, as it provides the specialist with complete visual control over the progress of the operation.

ADVANTAGES AND DISADVANTAGES OF OPEN RINOPLASTY

Open rhinoplasty implies the presence of marginal incisions performed along the edge of a large wing cartilage, as well as a columella incision. After performing the incisions, the skin is diverted to the bridge of the nose, exposing the bone-cartilage tissue, on which all the manipulations are performed.

The predominant features of open nose rhinoplasty include:

• the possibility of visual control, allowing the surgeon to monitor the progress of the operation and, if necessary, make any adjustments;
• the absence of stretching or eversion of tissues during surgery, which allows you to accurately match the tissue
• at the end of all manipulations and suture;
• the ability to install and fix grafts of complex shape, with strong anatomical defects;
• the ability to achieve perfect nose symmetry during surgery.
• It is important to note that, despite the many advantages, using open rhinoplasty is not always advisable. Like
• any other surgical intervention, it has not only advantages, but also disadvantages. So, for example, they include:
• higher invasiveness of the operation, providing a long rehabilitation period;
• malnutrition of the skin during surgery, due to the fact that the columellar arteries are cut;
• less predictable result compared to closed rhinoplasty.

Qualified plastic surgeons have various nasal rhinoplasty techniques and in each case offer the patient the most optimal way to solve the tasks.

Indications for open nose rhinoplasty

Open rhinoplasty is indicated in cases where it is impossible to achieve a given result by other methods. So, for example, the use of this technique is justified when:

• repeated rhinoplasty, when it is necessary to eliminate the shortcomings and errors of the previous operation;
  the installation of transplants with the aim of changing the shape, increasing the size of the wings of the nose, correcting congenital or acquired defects;
• complex anatomical configuration of the nose, when the use of other methods of correction is impossible.

It is also important to know that the correction method can be changed during the operation. Sometimes it happens that during a consultation the doctor offers the patient a closed operation, but already in the process of intervention he sees that it will not be possible to achieve the set goals using this technique. In this case, during the operation, a standard marginal incision can be made, allowing to expose the bone-cartilage tissue.

Summing up, it must be said that open nose surgery is performed when there are no other ways to solve the problem. However, this does not mean that this method is worse than others, since it also has many advantages, the most important of which is the possibility of acquiring a nose that is fully consistent with the wishes of the patient.
